Abstract

Oculomotor nerve supplies all extraocular muscles (medial rectus, inferior oblique, superior rectus, and inferior rectus) except superior oblique and lateral rectus. Thus, isolated medial rectus palsy only in a healthy individual is very rare and may indicate underlying systemic pathology. Here we present such rare and only case report of a healthy young female who presented with sudden onset deviation in her left eye and on investigations was detected to have mid-brain lesion. Thus, extraocular muscle palsies in a young patient needs thorough investigation to rule out any causative systemic disorder so that early intervention can be sought.
