Dysregulated responses to stress and alcohol cues in cortico-striatal-limbic regions in social drinkers with early trauma.

Abstract

Early trauma (ET) is associated with negative health outcomes, including elevated risk for stress- and alcohol-related disorders. Previous research highlights stress-related neural correlates of ET. However, neural correlates of ET during alcohol cue response remain understudied, particularly in social drinkers. The present study seeks to examine neurobehavioral responses to stress and alcohol cues in social drinkers with and without ET, aiming to elucidate neural mechanisms linking ET and risk for future alcohol misuse. Sixty-four social alcohol users completed a baseline assessment, eligibility determination, ET classification, and an emotion provocation task involving stressful, alcohol-related, and neutral cue exposure during functional magnetic resonance imaging (fMRI) scanning. ET individuals showed significantly greater levels of stress and alcohol craving ratings at baseline compared to NT individuals. During stress cues, ET individuals exhibited decreased activity in regulatory prefrontal regions (ventromedial prefrontal cortex (PFC), dorsolateral PFC), but hyperactivity in regions of emotional reactivity (amygdala, hippocampus, and putamen). During alcohol cues, ET individuals showed decreased activity in reward processing regions, including the dorsolateral PFC, anterior PFC, and putamen, but increased activity in the posterior cingulate cortex. Findings indicate that ET individuals exhibit neural responses characterized by decreased prefrontal regulation and altered activation in limbic-striatal subcortical regions during stress and alcohol cues, suggesting dysregulated control over stress and reward processing which may contribute to ET's increased risk for stress and reward-related disorders found in the general population.
